An inquiry into former nurse Lucy Letby's conduct at Countess of Chester Hospital is expected to identify failures in the hospital's patient safety procedures. Letby was convicted in 2023 of murdering seven infants and attempting to murder six others while working in the hospital's neonatal unit. The inquiry, led by judge James Thirlwall, examined hospital management and oversight during the period when Letby worked there.
