Workers at an Ohio job site informed supervisors that a trench exceeded safe depth limits before a cave-in killed Jose Alberto Barajas Martinez, according to an Occupational Safety and Health Administration report. OSHA stated that workers were instructed to enter the trench minutes after raising the depth concern. Martinez died in the incident.
